Responsibilities
- Perform comprehensive speech-language evaluations for residents in a long-term care setting and establish clinical diagnoses.
- Develop, implement, and monitor individualized treatment plans to improve communication, swallowing, and cognitive-communication goals.
- Document assessment findings, progress notes, and discharge plans in the electronic medical record (Epic/EMR) and relevant PDPM documentation.
- Collaborate with nursing, rehabilitation therapy, social work, and medical staff to optimize resident outcomes and ensure coordinated care.
- Provide patient and family education on communication strategies, feeding safety, and community resources; adjust goals as needed based on progress and changes in condition.
- Adhere to regulatory requirements, maintain licensure and professional standards, and participate in ongoing quality improvement related to speech-language pathology services.

Requirements
- Education: Graduate of an American Speech-Language-Hearing Association (ASHA)–accredited program with a degree in Speech-Language Pathology.
- Licensure: Current New York State license as a Speech-Language Pathologist.
- Certification: Eligible for or holds the Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) from ASHA.
- Experience: Must have long-term care (LTC) experience.
- Skills: Proficient with MS Office and Epic/EMR; knowledge of the PDPM reimbursement system.
- Eligibility: Ability to work in a LTC setting; travel assignment suitable for a licensed clinician.
