The role includes reviewing histories, medications, assessments, care plans, orders and referrals. You will oversee monthly care plans and the hours logged by care advocates, providing clinical oversight for each patient's health and social needs. Documentation is in Healthie using built-in templates, with a Monday–Friday daytime schedule and no on-call nights or #J-18808-Ljbffr
