Home Health Registered Nurse (RN) Responsibilities Providing services in accordance with the plan of care Making the initial evaluation visit and regularly reevaluating the patient's nursing needs, as well as ensuring the plan of care needs are met and making any necessary revisions Monitoring and evaluating patients' progress and updating care plans accordingly Educating patients and their families on managing the conditions and promoting wellness Informing physician and other personnel of changes in the patient's condition and needs, as well as collaborating with interdisciplinary healthcare team members to coordinate the best possible care Taking on‑call rotation for weekends, and holidays as assigned Participating in in‑service programs, supervising and teaching other nursing personnel as necessary Requirements Valid Registered Nurse (RN) license in the state of Illinois Minimum of one year of nursing experience, preferably in home health or a related setting Proficient in using electronic medical record systems, such as Axxess Previous experience in geriatrics and home health Ability to work independently and prioritize tasks effectively Ability to provide care with empathy, respect, and understanding Strong communication and interpersonal skills Benefits 401(k) with employer contribution or match Comprehensive medical, dental and vision insurance Paid time off (PTO), including vacation, sick time and holidays Mileage reimbursement for patient visits Flexible scheduling with manageable caseloads Paid orientation and ongoing clinical training Stable, full‑time hours with consistent patient assignments Medical Specialty Geriatrics Home Health Internal Medicine Orthopedics Physical & Rehabilitation Medicine Wound Care Experience Nursing: 1 year (Required) #J-18808-Ljbffr Center Of Illinois Home Healthcare
