Transitional Care Manager — Home Health Liaison (Travel)

BAYADA Home Health Care is seeking an experienced clinician to serve as Transitional Care Manager (TCM) in the Chester County, PA territory. You will coordinate initiation of home health services, liaise with discharge planners, case managers, clients and families to ensure seamless transitions and high-quality care.
