Travel Physical Therapist - $2,175 per week

Position Overview Centra Healthcare Allied is seeking a travel Physical Therapist for a contract assignment in Milwaukee, Wisconsin. Job Details Specialty: Physical Therapist Discipline: Therapy Start date: ASAP Duration: 13 weeks Hours: 40 per week (8‑hour days) Shift: 8 hours per day Employment Type: Travel Responsibilities Assess referred patients requiring rehabilitation services. Provide direct patient care and assess medical condition, functional capabilities, limitations, restrictions, and rehabilitation potential. Establish and administer a treatment program with goals tailored to the patient’s capacity and tolerance under physician direction. Set realistic and achievable goals, document and record patient condition, and educate patients and families on appropriate physical therapy methods. Qualifications Graduated from an accredited school. Current state license as a Registered Physical Therapist/ PT or actively pursuing licensure. Current CPR certification. Develop a plan of care for each patient. Provide skilled physical therapy services/interventions in accordance with physician orders. Assess patient needs, plan, evaluate, and modify care to meet goals of physical therapy interventions. Collaborate with all disciplines to plan and evaluate team goals for each patient.
