Mental Health Case Manager needed in Oklahoma City, Oklahoma to serve adults ages 18-64 as a collaborative member of the clinical care team, provide advocacy, coordinate care, navigate community resources, develop case plans, maintain patient relationships, support interdisciplinary care, and lead the growth of the case management program in alignment with the mission of the clinic. Applicants must have the minimum of a Bachelor of Science degree in Psychology or a related field plus at least one year experience as a mental health case manager including client assessment and intake, crisis intervention, coordination of care with treatment teams, and use of Epic and Volgistics systems.
