Licensed Practical Nurse (LPN)

Signature HealthCARE is seeking a compassionate, clinically skilled Licensed Practical Nurse (LPN) to join our Healthcare & Medical Services team. As an LPN with Signature HealthCARE, you will play a key role in delivering high-quality, resident-centered care in a collaborative, innovative environment focused on revolutionizing the healthcare experience. You will provide direct nursing care to residents and patients, including completing focused assessments, monitoring vital signs, administering medications and treatments within your scope of practice, and accurately documenting care in the electronic health record. Working closely with registered nurses, physicians, therapists, and other members of the interdisciplinary team, you will help implement individualized care plans that promote comfort, safety, functional independence, and overall well-being. In this role, you will support residents with daily clinical needs such as wound care, catheter care, blood glucose monitoring, and infection-prevention practices, while responding promptly to changes in condition and communicating critical information to the care team and families. You will educate residents and their families about diagnoses, medications, and care routines, reinforcing Signature HealthCARE’s commitment to resident-centered service and meaningful community impact. You will follow established nursing standards, facility policies, and state and federal regulations to ensure quality outcomes and survey readiness at all times.
