Nurse Practitioner for In-home Visits

Position Board‑certified Nurse Practitioner contractors are needed to conduct In‑Home Health Evaluations for Medicare Advantage, Medicaid, and certain commercial plan members. Key Responsibilities Conduct in‑home visits that focus on the member’s overall health and well‑being and identify potential barriers to care. Perform a medication and medical history review and a physical evaluation in the member’s home. Use pre‑populated, patient‑specific forms for each evaluation; do not prescribe medicine, order lab tests, or alter current treatment regimens. Pre‑scheduled visits with health plan members, no requirement for nights or weekends. Compensation and Incentives Earn up to $2,500 to $4,000 each week (average 6–9 visits daily, $80 per completed assessment). Per‑visit rates can increase up to 160% or 200% depending on working counties through a Market Multiplier. Additional compensation is available via a pay‑per‑test model for eligible visits. Availability incentives are based on weekly availability. Schedule Shifts are available Monday to Sunday. All work is on a short‑term or long‑term basis with a flexible schedule. No night or weekend shifts are required. Qualifications Board‑certified Nurse Practitioner. State license required. Independent contractor status (1099). Benefits Malpractice coverage.
