Qualifications:
- Associate Degree or higher
- Social Service Experience Preferred
- Certified Dementia Practitioner
- Management Experience in Long Term Care Preferred
Job Responsibilities:
- Complete MDS and other needed assessments
- Manage staff and day to day operations on memory care unit
- Run care plan meetings for residents on memory care unit
- Ensure daily programming is taking place
- Assist with daily programming when needed
- Work with other department leaders as needed to ensure smooth operation of the memory care unit.
- Educate staff on dementia unit related to dementia and programming
- As well as other duties that arise
