Description

• Conduct thorough evaluations for patients in both inpatient and outpatient contexts and create personalized treatment plans
• Implement physical therapy interventions aimed at mobility, strength, balance, and functional recovery
• Work closely with a multidisciplinary team of nurses, doctors, case managers, and rehabilitation professionals to enhance patient outcomes
• Educate patients and their families about therapeutic objectives, planning for discharge, and at-home exercise regimens
• Maintain accurate documentation of assessments, treatment plans, and patient progress according to institutional and regulatory standards

Education

• Doctorate in Physical Therapy (DPT) or equivalent from an accredited institution

Certifications

• Current state licensure as a Physical Therapist
• CPR certification preferred
