Travel Home Health Registered Nurse

Seeking a Travel RN for a home health setting responsible for providing comprehensive in-home patient care, care coordination, and skilled nursing services. This role requires strong independence, clinical judgment, and proficiency in home health documentation and regulatory compliance.

Location: Owensboro, KY
Schedule: Monday-Friday, 8:00 AM - 4:30 PM
Employment Type: Temp for a minimum of 3-months (likely extension)

Pay Rate: $ 55.92/hr.


Key Responsibilities

• Conduct comprehensive patient assessments in home settings
• Develop, implement, and manage individualized plans of care
• Perform skilled nursing interventions including:
  
  • Wound care and wound vac management
  • Catheter care
  • Medication administration and management
  • Injections and IV antibiotic therapy
  • PICC line care and maintenance
• Patient and caregiver education
• Disease management and care coordination
• Ensure accurate and timely clinical documentation
• Maintain patient safety and risk reduction practices
• Ensure compliance with regulatory and Medicare home health standards

Required Qualifications

• Active RN license
• Current CPR certification
• Valid driver's license
• Ability to work independently in patient homes
• Strong communication (verbal and written)
• Basic computer skills and EMR proficiency

Preferred / Nice to Have

• Prior home health experience
• Experience with Homecare Homebase (HCHB)
• Proficiency with OASIS assessments (SOC, ROC, recertification, transfers, discharge)
• Knowledge of Medicare home health regulations
• Strong documentation and scheduling coordination skills

Additional Details

• Productivity expectation: 30 points/week minimum
• Coverage area includes Owensboro, Ohio County, and Madisonville
