Physical Therapist (PT) - Part-Time

Location: Stevens Point, WI
Facility: Stevens Point Health Services
Setting: Skilled Nursing Facility (SNF)

Continuum Therapy Partners is seeking a compassionate and dedicated Physical Therapist (PT) to join our rehabilitation team at Stevens Point Health Services in Stevens Point, Wisconsin.

This part-time Physical Therapist opportunity offers the chance to provide meaningful patient care in a supportive, collaborative skilled nursing facility where your clinical expertise makes a daily impact.

As a Physical Therapist (PT) , you will evaluate and treat patients while helping improve mobility, strength, balance, endurance, and functional independence.

• Evaluate patients and develop individualized physical therapy treatment plans.
• Provide skilled physical therapy services in a skilled nursing facility setting.
• Perform therapeutic exercises, gait training, mobility training, and functional activities.
• Monitor patient progress and modify treatment plans as needed.
• Collaborate with physicians, nursing staff, caregivers, and interdisciplinary rehabilitation team members.
• Complete accurate and timely documentation according to company, state, and federal guidelines.
• Educate patients and families on mobility strategies, safety techniques, and home programs.
• Provide compassionate, patient-centered physical therapy services.

• Graduate of an accredited Physical Therapy program.
• Current Wisconsin Physical Therapist (PT) license required.
• Skilled nursing or rehabilitation experience preferred but not required.
• New graduates are encouraged to apply.
• Strong communication, organization, and teamwork skills.
