Advanced Practice Provider (APP) The Advanced Practice Provider promotes and maintains health by providing medical services to Sunrise Community Health patients under the supervision of a Physician. The Advanced Practice Provider identifies short-term and long-term patient care issues that must be addressed, provides information and prescriptions, recommends options, and courses of action, and implements Physician directives. The Advanced Practice Provider rotates between clinics for coverage. This is a hybrid Provider Position. - Provide direct patient care in an outpatient setting.
- Diagnose, treat, and prescribe medication.
- Work autonomously or in collaboration with other members of the healthcare team.
- Assess patient health by interviewing patients, performing physical examinations, reviewing medications, and reviewing medical histories.
- Manage acute, chronic illnesses and mental health.
- Write prescriptions, order diagnostic testing, and place referrals for patients.
- Interpret and evaluate patient data to determine patient management and treatment options.
- Institute emergency measures and emergency treatment or appropriate stabilization measures in situations such as cardiac arrest, shock, hemorrhage, and convulsions.
- Document patient care services by charting in patient and department records.
- Perform procedures by performing injections, skin biopsies, suturing, casting/splinting, and managing wounds.
- Instruct and counsel patients by describing therapeutic regimens, growth and development information, discussing family planning, providing counseling on emotional problems of daily living, promoting wellness and health maintenance.
- Provide continuity of care by developing and implementing patient management plans.
- Provide "On-Call" coverage as scheduled.
- Review and respond to patient concerns, reviews patient satisfaction surveys.
- FTCA requirements - Comply with requests for information in support of FTCA malpractice coverage.
- Adhere to clinical guidelines, performance of PCMH/QI measures.
- Maintain safe and clean working environment by complying with procedures, rules and regulations.
- Protect patients and employees by adhering to infection-control policies and protocols.
- Comply with federal, state, and local legal and professional requirements by studying existing and new legislation, anticipating future legislation, enforcing adherence to requirements, advising management on needed actions.
- Maintain professional and technical knowledge by attending educational workshops, reviewing professional publications, establishing personal networks, participating in professional societies.
- Maintain up-to-date license, DEA, board certification, and CME requirements for the position.
- Team Based Care - Clinical Care Collaboration
- Work in collaboration and continuous partnership with providers, clinical staff, and community resources in a team approach to promote access to appropriate care in the goal of enhancing patient health.
- Pre-Visit Team Based Planning
- Review daily schedule and chart by care team.
- Documented instructions are written on the schedule, typed into the EHR appointment note, or in the EHR as a flag or clinical list update.
- Ensure patient visit is more efficient by identifying:
- Records from transitions of care are available during the patient visit.
- Results from labs or diagnostic are in the EHR.
- Labs or other services that are not specified in the standing orders protocols.
- Master's Degree / Bachelor's Degree (B.S.) from a four-year college or university in a medical related field. Previous experience in the medical field as an Emergency Medical Technician (EMT) or Registered Nurse (RN).
- License to practice as an Advanced Practice Nurse and/or Physician Assistant in the State of Colorado.
- Minimum 3 years clinical related experience and/or training, or the equivalent combination of education and experience.
- Bilingual English/Spanish Preferred.
