Director of Social Services - McGregor PACE

McGregor PACE, (Program of All inclusive Care for the Elderly) We are a comprehensive, community-based healthcare program that helps older adults remain safely and independently in their homes and communities. PACE brings medical care, social services, therapy, behavioral health, nutrition, transportation, home care, and other supports together through an interdisciplinary team.

We are seeking a Director of Social Services who is a MSW and licensed as either LSW or LISW in the state of Ohio to lead and develop our Social Services team.

This is an opportunity to lead clinical practice, develop staff, build community partnerships, and influence organizational strategy while making a meaningful difference in the lives of older adults and their families.

Key Responsibilities

• Supervise, mentor, coach, and develop Social Services staff.
• Direct and oversee clinical social work practice, workflows, and documentation.
• Provide leadership and guidance for complex participant and family situations.
• Ensure compliance with federal and state regulations, PACE requirements, organizational policies, and professional standards.
• Ensure the social and emotional needs of participants and families are incorporated into care planning and goal achievement.
• Develop and monitor departmental quality improvement initiatives.
• Build relationships with community partners, including social service agencies, housing providers, nursing homes, assisted living communities, and group homes.
• Collaborate with the interdisciplinary team to support participant-centered care and positive outcomes.
• Participate in organizational strategy, planning, and leadership as a member of the McGregor PACE Administrative Team .
• Report directly to the CEO of McGregor PACE .

Qualifications

• Master's degree in Social Work from an accredited program.
• Current Ohio social work licensure (LSW or LISW).
• Progressive social work experience, including supervisory or leadership experience.
• Experience working with older adults and/or individuals with complex medical or psychosocial needs.
• Strong leadership, communication, problem-solving, and organizational skills.
• Experience in PACE, healthcare, long-term care, behavioral health, or senior services preferred.
