Physical Therapist – Home Health in Red Lodge, Montana. Responsibilities
- Conduct comprehensive PT evaluations and design goal-directed, individualized treatment plans to improve function and mobility
- Implement evidence-based interventions, including therapeutic exercise, gait training, balance, ROM, and functional retraining
- Reassess patient progress, adjust goals, and document outcomes in accordance with regulatory and facility standards
- Collaborate with physicians, nurses, and the home health team to coordinate patient care and discharge planning
- Educate patients and caregivers on home exercise programs, safety, and progression to optimize function outside direct care visits
- Maintain accurate, timely documentation and ensure compliance with HIPAA and relevant guidelines

Requirements
- Active Physical Therapist license in Montana or eligible for licensure by endorsement
- Doctor of Physical Therapy (DPT) degree or equivalent required
- 1+ year of clinical PT experience; home health or hospital experience preferred
- Ability to travel to patient locations within the service area and maintain a valid driver's license with acceptable driving record
- Strong communication, teamwork, and documentation skills
- CPR/BLS certification preferred or as required by local policy
