Description & Requirements

Territories are assigned to help minimize travel while allowing you to provide essential services throughout your community.

Essential Duties and Responsibilities:

• Complete medical and/or behavioral health assessments in accordance with contract requirements involving broad clinical complexity and variability; escalate complex and non-standard cases that are exceptions to policies.
• Conduct on-site, video, and/or telephonic assessments based on program requirements to determine an individual's needs for services and supports, eligibility, and level of care based on established guidelines; interpret clinical and program guidelines to support accurate and consistent determinations.
• Perform all job duties in compliance with Person First standards, HIPAA guidelines, and company confidentiality policies.
• Serve as an internal resource to team members within established processes.
• Travel to client, consumer or program locations to perform in-person assessments exceeding 50% of the workweek in support of program delivery.

Minimum Requirements:

Specific clinical license and experience requirements based on contract requirements.

Travel on-site to client, consumer or program locations for more than 50% per week required; specific travel requirements based on program contract requirements.
