Nurse Practitioner (NP) / Physician Assistant (PA) - Post-Acute, East Valley, AZ Location:Mesa, AZ (Primary) | Some coverage in Chandler Setting:Skilled Nursing & Assisted Living Facilities Schedule:Full-time, weekdays on-site9 AM – 5 PM, plusweekend rotation once or twice per month Employment Type:1099 Independent Contractor Compensation:Productivity-based pay with potential to earnup to $300,000 annually About the Opportunity We’re hiring a full-time Nurse Practitioner or Physician Assistant to provide ongoing care within post-acute and long-term care settings acrossMesa and Chandler. This position offers steady weekday hours, autonomy, and a manageable patient load in a supportive clinical environment. Providers typically see15–16 patients per dayin Skilled Nursing and Assisted Living communities, focusing on established, stable residents and new admissions as needed.
