Job Description:


- Treat illnesses and injuries through targeted physical exercises and therapies.
- Develop and revise personalized care plans based on assessments and patient needs.
- Provide preventive and rehabilitative procedures tailored to clients of all ages.
- Use various physical agents and advanced equipment, such as whirlpool baths and ultrasound, to deliver effective treatments.
- Stay compliant with Medicare, State, Federal, JCAHO, insurance, and local policies during documentation and care.
- Maintain confidentiality while documenting all skilled care accurately and timely.
- Supervise Physical Therapy Assistants and Home Health Aides as needed, ensuring adherence to regulations.
- Engage in staff meetings and case conferences to promote collaboration and meet community needs.
- Deliver exceptional customer service to all clients and stakeholders.
- Participate actively in quality management and continuous learning programs.

Requirements:


- Minimum of 2 years of experience in physical therapy.
- Must hold relevant general certifications (ACLS, BLS/BCLS, PALS, NRP).
- Ability to communicate effectively and build relationships across teams and with clients.
- Willingness to work 8-hour shifts, Tuesday through Saturday.
