Position Overview

Aviata Health Group is seeking a compassionate and experienced Social Services Coordinator to join our team. This role is responsible for supporting residents and their families by addressing psychosocial needs, facilitating care planning, and ensuring access to appropriate community resources. The ideal candidate is empathetic, organized, and committed to enhancing the well-being and quality of life of those we serve.

Key Responsibilities

• Assess residents' psychosocial needs, including emotional, social, and financial factors
• Develop and implement individualized care plans in collaboration with interdisciplinary teams
• Provide counseling and emotional support to residents and families
• Assist with discharge planning and coordination of community resources
• Advocate for residents' rights and ensure compliance with state and federal regulations
• Maintain accurate and timely documentation of services and interactions
• Participate in care conferences and facility meetings
• Support crisis intervention and conflict resolution when needed

Qualifications

• Bachelor's degree in social work, Human Services, or a related field preferred.
• Minimum of 1-3 years of experience in healthcare, long-term care, or social services setting
• Knowledge of federal and state regulations related to social services in healthcare
• Strong interpersonal, communication, and organizational skills
• Ability to work effectively within a multidisciplinary team
• Licensure (if required by state) preferred
