Compensation Routine Skilled Nursing Visits: $80 per visit Start of Care (SOC) Visits: $160 per visit Initial Evaluations, Recertifications, Re‑Evaluations, Resumption of Care, and Discharge Visits: $120 per visit Orientation and Onboarding: $50 per hour Additional compensation may apply for visits that exceed standard visit durations. Special Consideration We are especially interested in Registered Nurses with experience completing OASIS Start of Care (SOC) assessments. Additional opportunities may be available for clinicians experienced in SOC admissions, recertifications, OASIS documentation, and other Medicare home health assessments. Responsibilities Conduct skilled nursing visits in patients' homes Complete comprehensive assessments and reassessments Develop and update individualized care plans Provide patient and family education Perform wound care, medication management, home infusions, disease management and other treatments Coordinate care with physicians and interdisciplinary team members Monitor patient progress and communicate changes in condition Complete accurate and timely clinical documentation Ensure compliance with Medicare home health standards and agency policies Qualifications Active Hawaii Registered Nurse (RN) license Minimum one year of clinical nursing experience Home health experience preferred but not required Experience with OASIS SOC assessment preferred Strong assessment, clinical judgment, and documentation skills Ability to work independently and manage a patient caseload Reliable transportation and willingness to travel within the East Honolulu service area Strong communication and care coordination skills #J-18808-Ljbffr Always Best Care Senior Services
