This role leads the organization’s Value-Based Performance (VBP) strategy and serves as the central accountability point for clinical outcomes, risk identification, and system-wide intervention.This role is directly accountable for the development, training, rollout, and sustained execution of facility-specific VBP work plans and other corporate clinical initiatives, ensuring alignment between corporate strategy and facility-level performance.Reporting to the SVP of Administrative and Clinical Services, this leader provides direct oversight of Corporate Nurse Consultants and partners closely with Operations, Compliance, and facility leadership to ensure consistency, survey readiness, and high-quality resident outcomes across all centers.Duties and ResponsibilitiesClinical Performance & Value-Based Performance (VBP) Lead the organization's VBP strategy across all facilities, develop facility-specific work plans, and train facility leadership (NHAs, DONs, IDT) on metrics and execution. Drive improvement in hospitalizations, Discharge to Community, Discharge Function Scores, Quality Measures, and Five-Star performance. Prepare and present performance dashboards for executive leadership.Risk Management & Clinical Oversight Identify and mitigate clinical and operational risks including falls, high-risk residents, infection trends, and adverse events. Lead root cause analysis for hospital transfers and negative trends, and establish standardized high-risk review processes across all facilities.Survey Readiness & Regulatory Compliance Oversee system-wide survey readiness, trend and analyze survey tags, conduct mock surveys and audits, and partner with facility leadership on plans of correction. Ensure alignment with CMS regulations and clinical standards.Leadership of Clinical Consultants Supervise Corporate Nurse Consultants, set performance expectations, standardize workflows and reporting, and mentor clinical leadership across the organization.Collaboration & Cross-Functional Alignment Partner with Compliance, Operations, and interdisciplinary teams (nursing, therapy, admissions, reimbursement). Participate in QAPI, High-Risk, UR, and executive leadership meetings.Data Analytics & Reporting Maintain performance dashboards, analyze facility trends, deliver actionable insights to executive leadership, and support VBP-tied incentive program development.Policy & Standardization Develop and implement standardized clinical protocols, ensure consistent execution of key processes, and refine policies to meet regulatory and operational needs.Active RN license, in good standing; multi-state licensure preferredBachelor’s Degree required, Master’s Degree in Nursing, Healthcare Administration, or related field preferredMinimum 10 years of progressive nursing leadership, with at least 5 years in senior-level or multi-facility/post-acute leadership.Demonstrated experience overseeing multi-site clinical operations.Deep working knowledge of Skilled Nursing Facility (SNF) Operations, CMS Regulations and Survey Process, Quality Measures and Value-Based Purchasing Programs, and Clinical Performance Improvement and Risk Management. #J-18808-Ljbffr Cadia Healthcare
