Responsibilities
- Conduct initial and ongoing physical therapy evaluations for residents in the SNF setting.
- Develop and implement individualized treatment plans addressing mobility, strength, balance, and functional goals.
- Deliver therapy sessions, document progress, and adjust plans based on patient response.
- Collaborate with nursing, therapy aides, and other members of the interdisciplinary care team.
- Educate residents and families on home programs, safety, and prevention strategies.
- Ensure timely, accurate documentation and adherence to facility policies and regulatory requirements.

Requirements
- Current, unrestricted Physical Therapist license (verification and specifics to be confirmed)
- Experience in a skilled nursing facility or long-term care setting preferred
- Strong assessment, documentation, and communication skills
- Ability to work a 13-week travel assignment with guaranteed hours
- Availability to work the listed day shift (08:00–16:30)
