Registered Nurse - Case Manager - Bronx New York

About This Role The RN, Case Manager at CVS Health is responsible for leading care management activities, driving care coordination, and collaborating with interdisciplinary teams to ensure continuity of care for complex patients. This role is ideal for experienced nurses who are passionate about improving patient outcomes and have strong relationship-building skills. What You'll Do Manage an assigned caseload of complex patients in a value-based care environment. Accountable for panel metric performance in admission prevention and readmission prevention. Own overall care coordination for assigned patients as the primary point of contact. Manage transitions of care episodes, including clinical assessments and medication reconciliation. Collaborate with patients' PCP, family, Social Workers, and other care team members to evaluate needs. Ensure timely documentation of key clinical assessments after admissions. Lead in-person interdisciplinary care planning meetings. Perform timely nursing assessments and provide patient education for chronic condition management. Educate patients and families, empowering them in their care. Document visits in electronic health records according to internal standards. Requirements Education: Current RN license in assigned state required; Bachelor degree in nursing preferred. Experience: Minimum of 6-8 years nursing experience. 2+ years experience in transitional nursing, emergency room nursing, care coordination, discharge planning, or home health preferred. Licenses & Certifications: Certified Case Manager (CCM) required, or willingness to obtain within 12 months of hire.
