Physical Therapist - Travel Contract

Minimum Requirements

  • Bachelor‘s, Master‘s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
  • Active California Physical Therapist license required to start the assignment.
  • BLS certification from AHA or ARC may be required.
  • Minimum 1 year of home health experience with a Medicare-certified facility (client requirement).
  • Experience using HCHB (Homecare Homebase) EMR required (client requirement).
  • Productivity expectation: approximately 30 points per week (client requirement).
  • Ability to meet schedule expectations of a full-time, 40 hours per week assignment (client requirement).
  • Preferred: prior experience with Medicare documentation and home health outcomes tracking.



Assignment Details

  • Facility Type: Home Health.
  • Shift Name: Days.
  • Shift Details: Days. 8-hour shifts, Monday through Friday. Schedule example listed as 8:00 AM–5:00 PM.
  • Guaranteed hours per week: 40.
  • EMR: HCHB required.
  • Productivity requirement: 30 points per week.
  • Assignment length: 13 weeks.
  • Positions available: 1.
