Referral Coordinator - Registered Nurse

Job Overview This PRN role offers flexible, as‑needed hours within normal business hours (Monday–Friday, 8 am–5 pm), requiring 1-2 days weekly support to IL team needs. It involves coordinating with multiple hospital locations in IL and supporting hospice and home health services, with hospital experience a plus. Responsibilities Screen referrals for eligibility Verify insurance Obtain pre‑authorizations Track patient statuses Coordinate discharge planning Qualifications Applicants should have an RN license, 2-5 years of experience (preferably with some supervisory background), nursing or related degree.
