Description

American Traveler is seeking a licensed Physical Therapist with home health experience for a 20-week home health assignment in Dunedin, FL serving a diverse patient population including cardiopulmonary, geriatric, neuro, ortho, and oncology patients.

Details

• Home health setting with skilled home visits across Pinellas County, FL
• Patient population includes cardiopulmonary, geriatrics, neuro, ortho, and oncology
• Expected caseload of 6–8 home visits per day (minimum 6 points per day, with potential increase at manager's discretion)
• Day shift schedule, Monday–Friday 8:00 AM–5:00 PM, with every 3rd weekend required
• Visits and hours are self-scheduled based on the needs of the home care office
• Charting and documentation completed using BayCare Forms, BayCare Scheduling, and ACE Timesheet systems
• Equipment provided includes a laptop, MiFi for internet access, and a portable printer

Requirements

• Minimum 2 years of physical therapy experience required, with home health experience required
• Current BLS certification required
• Valid state PT license required if applicable
• Valid driver's license and personal vehicle required to conduct home visits
• OASIS training preferred
• Proficiency with home health equipment including crutches, gait belt, rolling walker, standard walker, sliding boards, soft/hard collars, shoulder slings, hospital bed, and supplemental oxygen delivery devices
• Skills required include assessment, cardiopulmonary/circulatory care, documentation, ortho, neuro, home exercise program development, medication awareness, patient and family education, safety and injury prevention, and recognition of signs and symptoms of physical, sexual, and psychological abuse
• Must be COVID-19 vaccinated or have an exemption form on file
• Flu shot required or declination on file; mandatory mask wearing at all times while at work if declining the flu vaccine
• 2 professional references from a supervisor within the past 2 years required for consideration

• Local candidates are accepted at the same rate as travelers
• Solid color scrubs required
• Primary responsibilities include conducting skilled home visits, patient and family education, development of home exercise programs, and thorough documentation
• 20-week contract assignment based in the Dunedin, FL area
