What You'll Do
  • Provide psychosocial assessments and develop patient-centered care plans
  • Support patients and families with crisis intervention and resource linkage
  • Collaborate with healthcare teams to coordinate services and discharge planning
  • Document clinical interactions accurately and promptly
  • Assist in case management and ensure compliance with regulatory standards
  • Participate in team meetings, patient rounds, and community outreach programs
  • Advocate for patient needs and promote holistic wellness strategies
What You Need
  • Current, unrestricted Louisiana Social Work License
  • Previous experience in healthcare or community-based settings preferred
  • Strong interpersonal and communication skills
  • Ability to work independently and as part of a multidisciplinary team
  • Compassionate attitude and commitment to high-quality patient care
