We are currently looking for field-based Population Health Advanced Practice Provider to join our Texas Team! Responsibilities: Complete patient visits across Bloom’s Texas territories (San Antonio and Dallas Fort Worth) with focus on gaps closure, discovery of previously unidentified complex conditions Use technology enabled chart discovery to highlight clinical gaps and improve outcomes Support teams with HEDIS gap closure, STAR rating success Demonstrate exceptional communication and relationship-building skills with care team members, including internal clinicians and external partners With support from Sr Director of Population Health, help identify opportunities and execute process improvement plans to improve population health outcomes Engage with and appropriately utilize population health data to drive superior patient outcomes at the practice level Attend ongoing educational opportunities Field Pop Health Provider will not hold their own patient panel, but will complete visits on others’ Bloom Primary Care patients.
