Community Health Advocate & Care Navigator

MyMichigan Health is seeking a Community Health Worker (CHW) for a grant-funded, one-year position to connect patients with health care, community, and social services addressing social determinants of health. The CHW will work with ambulatory patient-centered health teams and community partners to reduce health disparities.
