Kidney Care Advocate: Virtual Education & Navigation

Fresenius Medical Care is seeking a Kidney Care Specialist to educate CKD/ESRD patients, coordinate care, and support virtual patient engagement. The role may be field-based, virtual, or hybrid, with responsibilities spanning education, referrals, and collaboration with nephrology teams. Ideal candidates have 1–3 years in kidney care or chronic disease management, strong communication skills, and experience with CRM systems and EMRs.
