Position Summary This role requires full understanding and active participation in fulfilling the mission of Orange Coast Memorial Medical Center. The Social Worker (New Grad) provides social work services to patients and families in all hospital areas. The position requires seven days a week, including holidays, “on call” coverage on a rotating basis as determined by the Social Work Services Manager. Key responsibilities include conducting psycho‑social assessments, brief treatment, crisis intervention, patient education, discharge planning, and resource management. The Social Worker removes psycho‑social barriers to ensure safe and timely transition through the continuum of care and supports cost‑effective measures such as Performance Improvement Projects and LEAN activities. Essential Functions and Responsibilities Conduct psycho‑social assessments and interventions with patients and families. Address psycho‑social issues and concerns throughout the continuum of care. Navigate systems and advocate to assist patients in accessing appropriate resources and services. Make mandated protective services reports based on psycho‑social assessment of patient and family. Address barriers to a safe and timely discharge. Be present and punctual; required to be on campus during core business hours. Follow company policies, procedures, and directives. Interact in a positive and constructive manner with colleagues and patients. Prioritize and multitask effectively. Perform other duties as assigned. *Placement in the pay range is based on multiple factors including relevant years of experience and qualifications. Qualifications Minimum Requirements 1 year or less of direct social work experience in an acute healthcare setting preferred. Competency is defined and measured via the Social Worker Job Specific Competency Assessment and Checklist, with assessment at orientation and annually.
