Nurse Practitioner for our Skilled Nursing Facility (SNF)

Compensation: $105,000 - $130,000 Annual Salary

Position Description

We are looking for a dedicated Nurse Practitioner for our skilled nursing facility in Elizabethtown, KY. This is a full-time, in-person position, Monday through Friday during daytime hours, with no nights, no weekends, and no on-call. The Nurse Practitioner will manage a panel of long-term care and short-stay rehab residents, handling chronic disease management, acute changes in condition, and coordination with the facility's nursing and therapy teams.

Qualifications of the Nurse Practitioner:

Active and unrestricted Kentucky Nurse Practitioner license in good standing

Active DEA registration

Board certification as an FNP or AGNP

Experience in skilled nursing, long-term care, or geriatrics preferred

Responsibilities of the Nurse Practitioner:

Provide primary care to residents, including physical exams, assessments, and medication management

Diagnose and treat chronic and acute conditions within the NP scope of practice

Order and interpret labs and diagnostic tests, and adjust care plans accordingly

Collaborate with facility nursing staff, therapy, and the interdisciplinary team

Maintain accurate and compliant clinical documentation
