Responsibilities

• Provide direct patient care in the home
• Assist with assessments and care plans
• Administer medications and treatments
• Perform wound care and patient education
• Coordinate with nurses, physicians, and care team
• Maintain accurate documentation

Requirements

• Active LPN license (Georgia)
• CPR certification
• Reliable transportation
• Strong communication and organizational skills
• Home health experience preferred
