Registered Nurse Case Manager

The Registered Nurse Case Manager coordinates the management of care, ensuring optimal utilization of resources, service delivery, and compliance with external review agencies. This role provides ongoing support and expertise through comprehensive assessment, care planning, plan implementation, and overall evaluation of individual patient needs. The focus is on enhancing the quality of patient management and satisfaction while promoting continuity of care and cost-effectiveness through the integration of case management functions, utilization review, and discharge planning.

Responsibilities

• Coordinate management of care to enhance patient outcomes.
• Conduct comprehensive assessments and develop individualized care plans.
• Implement care plans and evaluate patient progress and needs.
• Serve as a patient advocate through resource utilization and discharge planning.
• Collaborate with interdisciplinary teams to ensure effective patient management.

Schedule

This position operates on a Monday to Friday schedule from 8:00 AM to 5:00 PM.

Required Experience / Certifications / Licensure

• Minimum 2 years of recent RN Case Management experience.
• Experience in LTAC or STACH settings preferred.
• Knowledge of MCG, discharge planning across acute care settings, and strong interdisciplinary team collaboration.
• BLS certification required (must be attached with the application).
• Texas RN or multi-state RN license required (must be attached with the resume).
