Conduct thorough evaluations of patients in both inpatient and outpatient environments, crafting personalized treatment plans based on individual needs. Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function. Collaborate closely with a multidisciplinary team, including nurses, physicians, case managers, and rehabilitation professionals, to ensure optimal patient outcomes. Provide education and support to patients and their families regarding therapeutic objectives, discharge procedures, and home exercise regimens. Maintain detailed documentation of evaluations, treatments, and progress in compliance with regulatory standards. Engage in care team rounds and interdisciplinary meetings to promote cohesive patient management.

Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution. Valid state licensure as a Physical Therapist. CPR certification is preferred. Specialized qualifications (such as neurological or geriatric certifications) are beneficial but not mandatory.

Previous experience in a clinical setting is highly preferred. Proficiency in treating patients with intricate medical conditions and mobility challenges. Excellent communication and teamwork abilities are essential. Comfort with digital health records and clinical documentation systems is necessary.
