Summary: This role involves obtaining and updating medication histories for patients admitted or pending admission, ensuring accurate medication reconciliation within the electronic medical record system. The position supports healthcare providers by compiling comprehensive medication lists to enhance patient safety and care quality.
Key Responsibilities:
- Navigate electronic medical records to prioritize and complete medication histories for admitted or pending patients.
- Gather medication information from diverse sources such as patient interviews, physician offices, community pharmacies, and other healthcare facilities.
- Compile detailed medication lists including drug name, dose, route, frequency, PRN indications, and last dose taken.
- Document medication histories and allergy information accurately and promptly in the electronic medical record.
- Refer unresolved medication issues to pharmacists and admitting providers.
- Maintain patient and staff confidentiality by securing records and releasing information only to authorized personnel.
- Report safety hazards promptly to supervisors or appropriate personnel.
- Conduct educational sessions for technicians, nurses, and other healthcare staff, including patient education.
- Precept and train pharmacy technicians, students, pharmacists, and nursing staff on medication history processes.
- Assist with data collection for performance improvement initiatives.
- Perform other duties as assigned.
Education and Formal Training:
- Completion of Pharmacy Technician Certification Board (PTCB) certification required unless currently enrolled in a PharmD program.
- Registration with the state Board of Pharmacy is mandatory.
Experience:
- 6 months to 1 year of experience as a pharmacy technician is required.
- Preferred: 2 years of retail pharmacy experience.
Knowledge, Skills, and Abilities:
- Strong technical proficiency with attention to detail.
- Familiarity with common medication doses and forms is preferred.
- Excellent interpersonal and communication skills to interact effectively with patients and healthcare professionals.
- Ability to prioritize tasks in a high-volume environment and adapt to changing workloads.
Physical Requirements:
- Ability to work independently in a sometimes stressful environment.
- Capability to read physician notes, medication labels, and lists accurately.
- Manual dexterity for operating pharmacy equipment such as computers, phones, fax machines, and automated dispensing cabinets.
- Ability to stand and walk for extended periods (6-10 hours) and lift moderate loads (up to 10 lbs), including stooping and kneeling.
- Effective verbal and written communication skills are essential.
This position is based in a regional healthcare setting serving the Southeastern United States.
