Assignment Duration

This is a 13-week contract position.

Schedule/Shift

This position primarily works Monday through Friday with rotating weekends every third weekend. The shift is from 8:00 AM to 5:00 PM. On-call rotations for overnight are required, with weekday calls starting at 5 PM and ending at 8 AM, and weekend on-call from Friday at 5 PM through Monday at 8 AM.

Job Description

The role involves managing a caseload of 14-18 patients, with an expected productivity of 25 visits per week. The focus is on routine visits, recertification visits, and admission visits in a hospice setting. RNs will utilize EPIC for documentation and are expected to collaborate effectively within a team.

Responsibilities

• Conduct routine visits, recertifications, and admissions for hospice patients.
• Manage a caseload of 14-18 patients, completing 5-6 visits daily.
• Utilize in-home infusion pumps; no ventilator or invasive monitoring required.
• Collaborate with team members and communicate effectively with patients and families.
• Participate in on-call rotations as required.

Required Experience

• At least 1 year of home health or hospice experience required.

Certifications/Licensure

• Current BLS certification is required.
