Job Summary

"Navitas Healthcare, LLC" is seeking an experienced Licensed Master Social Worker to provide psychosocial assessments, supportive therapy, and discharge planning services in an inpatient medicine/surgery setting. Role focuses on patient support, care coordination, and referral management.

Key Responsibilities

• Conduct psychosocial assessments for inpatient patients
• Provide supportive counseling and therapy
• Perform discharge planning and care coordination
• Make appropriate patient referrals to community resources
• Collaborate with interdisciplinary healthcare teams
• Document assessments and interventions in EMR
• Support inpatient medicine/surgery unit operations
• Ensure compliance with hospital social work standards

Required Qualifications

• Master’s Degree in Social Work (MSW)
• Active LMSW license (NYSED)
• Minimum 1 year LMSW experience
• Strong communication and documentation skills
• Experience in inpatient hospital setting preferred
