SSM Health Monroe Clinic Hospital in Wisconsin seeks a Diabetes Education specialist to assess learning needs, develop education plans, and deliver self-management education to patients and caregivers in inpatient or outpatient settings. You will collaborate with the care team and monitor outcomes to improve diabetes care delivery. Must complete BLS HCP within 90 days and participate in program oversight, maintaining qualifications and contributing to continuous improvement. #J-18808-Ljbffr
