Position Summary :

The Case Worker provides comprehensive case assistance services to older adults participating in SHOPP's Community Care Network Program (CCNP). The Case Worker assists participants in accessing benefits, entitlements, health and social services, and community resources that promote independence, stability, and overall well-being. Through assessment, advocacy, supportive counseling, referrals, and follow-up, the Case Worker empowers older adults to successfully navigate complex service systems while maintaining dignity, choice, and quality of life. This position also supports outreach efforts, participant engagement, documentation, and collaboration with community partners to ensure the delivery of high-quality, person-centered services.

Essential Functions and Responsibilities :

The following duties are representative of the essential functions of this role and are not exhaustive.

Role-Specific Responsibilities

  • Conduct comprehensive screenings and assessments to identify participants' needs, strengths, and eligibility for benefits, entitlements, and supportive services.
  • Provide case assistance services, including information, referrals, advocacy, supportive counseling, and service coordination for older adults.
  • Assist participants in completing applications for public benefits, health insurance, housing resources, transportation, nutrition programs, and other available services.
  • Monitor participant progress and conduct timely follow-up to ensure services have been received and participant needs continue to be addressed.
  • Maintain accurate, timely, and comprehensive case documentation in the agency's electronic reporting system and other required databases.
  • Develop individualized service plans that reflect participant goals, preferences, and identified needs.
  • Provide crisis intervention and appropriate referrals when urgent social, medical, housing, or safety concerns arise.
  • Conduct outreach activities and provide presentations on benefits, entitlements, available services, and healthy aging topics to participants and community partners.
  • Provide Services to SHOPP off-site locations including PopUp Café.
  • Recruit and enroll eligible participants into case assistance services and other SHOPP programs and activities.
  • Conduct telephone reassurance and wellness calls to assigned participants as appropriate.
  • Identify, establish, and maintain collaborative relationships with community agencies, government agencies, and other service providers.
  • Advocate on behalf of participants to facilitate access to services and reduce barriers to care.
  • Maintain confidentiality and ensure compliance with HIPAA, agency policies, and applicable regulations.
  • Monitor service utilization and assist in achieving program performance targets and contractual outcomes.
  • Assist with program initiatives as assigned.

General Responsibilities

  • Participate in supervision, team meetings, and organizational initiatives.
  • Maintain compliance with SHOPP personnel policies, procedures, and funder requirements.
  • Complete accurate and timely documentation, reporting, and administrative tasks.
  • Support program and organizational goals and meet established deadlines.
  • Demonstrate professionalism, accountability, and responsiveness in all aspects of work.
  • Work independently and collaboratively to support team and program success.
  • Demonstrate flexibility in supporting evolving program and organizational needs.

Qualifications :

Required:

  • Bachelor's degree in Social Work, Human Services, Gerontology, Psychology, Sociology, or a related field; or an Associate's degree with a minimum of three (3) years of experience providing case assistance or social services.
  • Experience working with older adults or other vulnerable populations in a community-based or social service setting.
  • Knowledge of public benefits, entitlements, healthcare systems, and community resources available to older adults.
  • Experience conducting assessments, providing referrals, and coordinating services.
  • Strong organizational, documentation, and case assistance skills.
  • Excellent verbal, written, interpersonal, and customer service skills.
  • Proficiency in Microsoft Office applications and electronic case recording systems.
  • Ability to maintain confidentiality and exercise sound professional judgment.

Preferred:

  • Bachelor's or Master's degree in Social Work (BSW/MSW).
  • Bilingual (English/Spanish).
  • Experience working within NYC Aging-funded programs or similar older adult service programs.
  • Knowledge of benefits and entitlement programs including SNAP, Medicaid, Medicare Savings Programs, SCRIE/DRIE, HEAP, and housing resources.
  • Crisis intervention and supportive counseling experience.
  • First Aid and CPR certification.

  • Problem Solving: Assesses participant needs, identifies barriers, and develops practical solutions while utilizing available resources.
  • Communication: Communicates clearly, compassionately, and professionally with participants, families, staff, and community partners through effective verbal, written, and interpersonal communication.

  • Ability to sit, stand, walk, bend, and move throughout the workday.
  • Ability to travel throughout the community to, satellite sites, community agencies, and partner locations as needed.
  • Ability to operate computers, office equipment, and electronic documentation systems.
  • Ability to lift and carry program materials and office supplies up to 20 pounds.
  • Ability to work in office settings, community centers, and various community locations.

Work Schedule and Travel :

  • Typical work schedule: Monday through Friday; 9 am to 5 pm.
  • Occasional evening or weekend hours may be required for outreach events, presentations, or community activities.
  • Local travel between SHOPP sites, satellite locations, and community partner agencies is required.
