In-Home Registered Nurse: Skilled Care & Care Planning

The Legacy Senior Communities seeks an experienced RN to provide skilled nursing care in a home health setting under the patient plan of care. You will assess, educate, and coordinate services to promote optimal outcomes for the client. You will conduct admissions, document thoroughly, and participate in discharge planning, while collaborating with physicians and other clinicians.
