LVN / LPN - Home Health

Job Description

Job Summary:
Provides professional home health nursing care under physician orders, coordinating patient treatment plans and ensuring quality care delivery. Supervises home health aides and licensed practical nurses to maintain compliance with agency policies and procedures.

Responsibilities:

• Coordinate and deliver nursing care in patients' homes using the nursing process.
• Supervise and evaluate performance of Home Health Aides and Licensed Practical Nurses.
• Document patient interactions and clinical activities accurately within required timeframes.
• Travel within designated territory to conduct start of care, follow-up, and wound care visits.
• Ensure compliance with agency policies, including documentation, confidentiality, and safety standards.
• Communicate effectively with physicians, patients, and team members regarding patient progress.

Qualifications:

• Possess a valid and current LVN/LPN license valid in the state.
• Have at least 1 year of recent adult home health or hospice experience.
• Proficient in using HomeCare HomeBase EMR, with recent usage preferred.
• Be comfortable traveling up to 75 miles from the home clinic.
• Hold a reliable vehicle, valid driver's license, and current auto insurance.
• Demonstrate excellent clinical, communication, and organizational skills.
