Triage Visit RN - Home Care

Triage Visit Registered Nurse - Home Care Hours Part-time, Weekend Days – Available every weekend or every other weekend to take calls, triage patients, and make visits as needed. Work remotely and travel to patient homes when applicable. General hours: 8:00 am – 5:00 pm Saturday and Sunday. Training Availability during weekdays, Monday‑Friday from 8:00 am – 5:00 pm for a minimum of two days per week over 2–6 weeks, depending on schedule. Responsibilities Take calls from the On Call Service and address them based on call type (e.g., Intake/Referrals, Scheduling, Patient Concern). Provide telephone triage and make patient visits as appropriate. Complete accurate and timely documentation of patient/family services provided. Provide education/instructions to the patient/family in accordance with the plan of care. Administer medications and treatments as prescribed by the physician and in accordance with nursing standards of practice. Benefits Hourly Pay Range: $38.46 – $46.86 Mileage Reimbursement 401(k) Scrubs Allowance Recognition Programs Locally owned and operated Qualifications (Must-Haves) Graduate of an accredited school of nursing and licensed to practice as an RN in Minnesota. License must be active and in good standing. Minimum of 12 months of accumulated RN experience within the last five years. Ability to travel within the geographic area serviced by the office, with valid driver’s license, reliable transportation, and proof of auto insurance. Current CPR Certification required.
