Overview
Licensed Practical Nurse, Long Term Care. Springfield, Vermont, United States. Responsibilities
- Administer prescribed medications and treatments per physician orders and facility policy.
- Monitor resident status, assess changes, and report findings to the charge nurse and physician as needed.
- Provide and document nursing care, including ADLs, vital signs, wound care, and injections as allowed by scope of practice.
- Collaborate with the interprofessional care team to implement and adjust individualized care plans.
- Maintain accurate and timely documentation in electronic or paper records, ensuring confidentiality and compliance with regulatory requirements.
- Respond to resident needs and call lights promptly, promoting safety, comfort, and dignity.

Requirements
- Active LPN license in Vermont (in good standing)
- Graduation from an approved practical nursing program (LPN diploma)
- Current CPR/BLS certification (or ability to obtain)
- Prior experience in long-term care or skilled nursing settings preferred
- Ability to work evenings and/or overnight shifts
- Ability to pass required background check and drug screening
