Minimum Requirements

• Bachelor‘s, Master‘s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active Illinois Physical Therapist license required to start the assignment.
• BLS certification from the American Heart Association or American Red Cross may be required.
• Experience working in skilled nursing facility (SNF) settings preferred.
• Ability to document skilled therapy services consistent with facility and payer requirements.

Preferred Qualifications

• Experience with geriatric populations and post-acute care.
• Familiarity with common SNF equipment and modalities used in long-term care and rehab.
