Required Qualifications
Physical Therapist, Rehab Licenses: PT-NY Certifications: BLS References: 1 Reference in entire work history
• Minimum of 1-2 year’s pediatric PT experience Required
• Completion of a pediatric clinical affiliation
Master’s Degree or Doctorate in PT
• NYS licensure and registration certificate
* NPI Number
• CPR Certification
Must be able to push, pull, move, and/or lift a minimum of fifty (50) pounds.
• Perform tasks that may involve exposure to the resident’s blood/body fluids.
• Must be able to assist in the evacuation of residents, if necessary.
