Hospice RN Case Manager — Compassionate Home Care Leader

- p Gentiva is seeking an experienced Hospice RN / RN Case Manager to be the primary contact for patients and families, delivering skilled nursing care in home settings and guiding comprehensive care plans. You will perform initial and ongoing assessments, adjust plans, participate in an interdisciplinary team, and provide education and bereavement support to loved ones.
- p The role requires RN licensure, CPR certification, and hospice or palliative care experience. A strong emphasis on documentation, regulatory compliance, on‑call availability, and commitment to compassionate end-of-life care is expected.
