The Family Nurse Practitioner (FNP) is responsible for providing comprehensive primary healthcare services to patients across the lifespan. Working collaboratively with physicians and other healthcare professionals, the FNP evaluates, diagnoses, treats, and manages acute and chronic medical conditions while emphasizing preventive care, patient education, and evidence-based practice. The Family Nurse Practitioner practices within the scope of state and federal regulations, organizational policies, and national standards of care.
Essential Duties and Responsibilities:
- Perform comprehensive history and physical examinations.
- Assess, diagnose, and manage acute and chronic illnesses.
- Order, perform, and interpret diagnostic tests including laboratory studies, imaging, and other diagnostic procedures.
- Develop and implement individualized treatment plans.
- Prescribe medications and therapies in accordance with state regulations and collaborative practice agreements, when applicable.
- Manage chronic diseases including but not limited to: Hypertension, Diabetes, Hyperlipidemia, COPD, Asthma, Heart failure, Thyroid disorders, Obesity.
- Provide preventive health services including: Annual wellness visits, Routine physical examinations, Immunizations, Cancer screenings, Health risk assessments.
- Perform routine office procedures within scope of practice.
- Counsel patients regarding lifestyle modification, nutrition, exercise, smoking cessation, and disease prevention.
- Coordinate referrals to specialists and ancillary services.
- Review laboratory and diagnostic results and communicate findings to patients.
- Maintain accurate, timely, and complete electronic medical records.
- Participate in quality improvement initiatives and population health programs.
- Collaborate with physicians, specialists, nursing staff, care coordinators, social workers, and other interdisciplinary team members.
- Respond to patient messages, prescription refill requests, and clinical questions.
- Participate in clinical meetings, continuing education, and organizational initiatives.
- Comply with HIPAA, OSHA, Joint Commission, CMS, and organizational policies.

Qualifications:
- Education: Master's or Doctorate degree from an accredited Family Nurse Practitioner program.
- Licensure: Current unrestricted Registered Nurse (RN) license. Current Advanced Practice Registered Nurse (APRN) license. Current Family Nurse Practitioner (FNP-BC or FNP-C) certification. Current DEA registration (or eligible). Illinois Controlled Substance License (if applicable). Current BLS certification. ACLS preferred.
- Experience: Preferred: 1–3 years of Family Nurse Practitioner experience. Experience in: Primary Care Family Medicine Internal Medicine Community Health Urgent Care Federally Qualified Health Centers (FQHC) Value-Based Care New graduates may be considered.
- Knowledge, Skills, and Abilities: Strong clinical assessment and diagnostic skills. Ability to formulate evidence-based treatment plans. Excellent communication and interpersonal skills. Strong organizational and time-management abilities. Proficiency with electronic health records (EHR), preferably Cerner. Ability to work independently and collaboratively. Commitment to patient-centered care. Knowledge of CMS quality measures and preventive care guidelines. Cultural competence and sensitivity to diverse patient populations.
- Physical Requirements: Ability to stand and walk for extended periods. Ability to lift up to 25 pounds occasionally. Ability to perform repetitive hand movements for documentation. Ability to respond to medical emergencies.
- Working Conditions: Outpatient medical office or ambulatory clinic. Exposure to infectious diseases and bodily fluids. Frequent interaction with patients, families, and healthcare professionals. Standard clinic hours with occasional evening or weekend coverage as needed.

- Complete documentation within 24 hours of patient encounters.
- Meet productivity expectations (e.g., patients per day/RVU targets as established by the organization).

- Maintain compliance with continuing education and certification requirements.
- Demonstrate professionalism, teamwork, and accountability.
Reports To: Medical Director, Director of Primary Care, or Supervising Physician (per organizational structure).
