Hospice Medical Social Worker (Master's Level)

Job Details Job Location: West Branch, MI 48661 The Care Team Hospice is seeking a compassionate and dedicated Part‑Time Hospice Medical Social Worker to join our team! About the Role As a Hospice Medical Social Worker, you will serve as a vital member of the interdisciplinary team, providing emotional support, psychosocial assessments, counseling, bereavement services, and community resource coordination. You'll work closely with patients, families, caregivers, and clinical team members to ensure each patient receives compassionate, holistic care that honors their goals, wishes, and quality of life. Key Responsibilities Complete psychosocial, emotional, spiritual, and bereavement assessments while developing and supporting individualized plans of care. Provide counseling, crisis intervention, emotional support, and advocacy for hospice patients and their families. Collaborate with the interdisciplinary team to identify patient and family needs, coordinate services, and support quality end‑of‑life care. Connect patients and families with community resources and support services while promoting continuity of care. Maintain timely and accurate documentation, participate in care planning meetings, and support bereavement and family services as needed. Qualifications Master's Degree in Social Work (MSW) preferred; Bachelor's Degree in Social Work or related field considered in accordance with state regulations. Previous hospice, home health, healthcare, or counseling experience preferred. Strong assessment, counseling, communication, and advocacy skills. Ability to work independently while collaborating effectively with an interdisciplinary team. Valid driver's license, reliable transportation, and current automobile insurance required.
