What you will do: Work closely with patients to assess their physical and psychosocial needs. Planning, development, and implementation of multidisciplinary plan of care. Evaluation of patients’ responses to care. Maintain patient records including initial evaluations/assessments, progress notes, and discharge summaries. What you will need: Graduate from an accredited program for Physical Therapy. Current licensure as a Physical Therapist in the State of WI. CPR Certification (or ability to obtain within 3 months).
